OBJECTIVE Provide the patient and / or family with the legal documentation necessary for the Hospital center looking for continuity of care. Material - Bag for waste. - High Volunteer Form (triplicate). - Printed of High. - Printed for ambulance. - Printed for Kitchen. - Printed for Pharmacy. - Medical report of discharge. - Nursing Report to the High. - Bed material closed. - Material of serum therapy, oxygen therapy, etc. According to the needs of the patient. - Medication and healing material if needed. Equipment - Dirty laundry cart. - Wheel cart or stretcher. - Bed set closed. - Therapy equipment, oxygen therapy, etc. According to needs. - Log book of the unit. PROCESS 1. Inform the responsible doctor. 2. Deliver the High Volunteer form to the most his sign. 3. Take the ambulance form. 4. Deliver Nursing report to discharge, reviewing with patient and family the Nursing care to be continued outside the hospital. 5. Deliver medication and curing material if necessary for continuity of treatment. 6. To dismiss the sick person and / or family. 7. Record in the Nursing Evolution sheet date and time of discharge. 8. Record the date and time of registration in the logbook. 9. To print forms of High and High voluntary to the service of Admission. 10. Fill in and print out forms for Kitchen and Pharmacy, if any. 11. Organize the Hª clinic to be sent to the archives service. 12. Pick up and clean room. 13. Make bed closed. 14.
