OBJECTIVE
Reincorporar the patient to his usual environment, seeking continuity of care.
Material
- Bag of waste.
- Printed for ambulance or taxi if the patient requires it.
- Printed for Kitchen.
- Printed for Pharmacy.
- Report of Nursing at discharge.
- Medical Report of High.
- Closed Bed Material.
- Medication and healing material, if need be.
- Part of High.
- Nursing records.
Equipment
- Dirty laundry cart.
- Trolley of wheels, or stretcher according to need.
- Equipment for Closed Bed.
- Log book of the unit.
PROCESS
1. Inform the discharge patient and family well in advance.
2. Take printed ambulance if you need.
3. Deliver nursing report to discharge, reviewing with patient and family the
Nursing care to be continued outside the hospital.
4. Deliver medication and necessary material, if necessary, for the continuity of the
Tto. During the first 24 hours.
5. To dismiss the patient and his family.
6. Record the Registration in the Unit Registration Book and in the
Nursing.
7. To print printed to the Admission Service.
8. Collect the medical record to send to the Archives Service.
9. Fill in and submit the form to Cook and Pharmacy (if any).
10. Pick up and clean room.
11. Perform closed bed procedure. (B-1).
12. Advise the Admission Service of bed availability for a new
patient.
OBSERVATIONS
- In Pediatrics and Special Services, the relative who collects the patient, has to
Identify yourself signing in the sheet of evolution of Nursing registering your number
Of DNI.
